Character Design Questionnaire: 1. What is your character's Name (First Name, Middle Name, Last Name)? 2. Give a physical description of your character: Please include such details as height, weight, hair color, eye color, gender, ethnicity, distinguishing features, handicaps (allergies or physical quirks), etc. Other pertinent details that might be included are blood type and date of birth. 3. Please give a basic description of your character's personality (At the beginning of the campaign). Please consider their general outlook on life, basic attitude, the way they approach problems, likes and dislikes, sexual orientation, ambitions, fears, mental quirks, etc. 4. Please describe your character's occupation and level of education. Also list hobbies, languages (spoken or read), areas of extra-curricular knowledge or activities, reasons for having special information or skills, etc. Military service? 5. Does your character have pets, or keep plants? 6. Describe your character's family; are they married, or have other romantic attachments? List parents, siblings or other important relatives. 7. What political leanings, if any does your character have? 8. What sort of vehicle(s) do(es) your character have the ability to operate/drive? 9. Give an estimate of your character's socioeconomic standing and approximate yearly income. 10. What type of dwelling your character have (Apartment /Duplex /House /Trailer /Vanagon /Condo /Tent /Homeless /etc)? How is it furnished? 11. Tell me about your character's typical weekday. 12. Describe any special "Contacts" or friends your character might want to call on, and how close they are (and why). 13. Does your character smoke, and if so, what? Does your character have any other politically incorrect habits (caffeine, alcohol, other drugs)? 14. Musical Preferences (More than one may be selected): a. Classical b. Folk c. Rock d. Pop e. Country/Western f. Jazz/Blues And so on... 15. Favorite foods? 16. Favorite forms of entertainment? a. Television b. Theatre c. Tormenting the helpless And so on... 17. Feelings on the occult: a. Occult...Is the a new band? b. All things occult are automatically seen as EVIL and/or the work of the Devil. (This can be used in conjunction with any of the following.) c. Your character has had their fortune told, or played with an Ouija board. d. Your character owns a tarot deck (or similar divination device) that has actually been used (although not necessarily by them). e. Your character has read a number of the classic works in the occult field and keeps abreast of the Occult News of the Day. f. Your character has an occult library, scrying devices, and so forth that are used frequently. g. Your character can define Oomancy and detail the fallacies therein. 18. As far as the supernatural: a. There is NO such thing as the supernatural. What a silly idea. b. All things supernatural are automatically seen as EVIL and/or the work of the Devil. (This can be used in conjunction with any of the following.) c. The X Files is a great show, but as nifty as it is, nothing supernatural exists. d. There *may* be a supernatural world out there, but there has never been any demonstrable evidence. e. 99% of all supernatural experiences are delusional, but as for the remainder... f. Your character is pretty certain that they've had some sort of supernatural experience before in their life. g. Your character firmly believes in magic/spirits/etc and *definitely* has had a supernatural experience in their life. h. NO thing exists that is not somehow supernatural in nature. 19. As far as UFOs: a. There is NO such thing as UFOs. Don't be stupid. b. All UFOs are automatically seen as EVIL and/or the work of the Devil. (This can be used in conjunction with any of the following.) c. The X Files is a great show, but as nifty as it is, UFOs don't exist. d. There *may* be life on other worlds, but there has never been any demonstrable evidence that aliens have been to Earth. e. 99% of all UFO experiences are delusional, but as for the remainder... f. Your character is pretty certain that they've had some sort of UFO experience before in their life. g. Your character firmly believes in Aliens/UFOs/etc and *definitely* has had a such an experience in their life. h. NO thing exists that is not somehow related to UFOs. 20. As far as Conspiracy Theories: a. There is NO such thing as Conspiricies. Don't be stupid. b. All Conspiricies are automatically seen as EVIL and/or the work of the Devil. (This can be used in conjunction with any of the following.) c. The X Files is a great show, but as nifty as it is, Such Conspiracies don't exist. d. There *may* be conspiracies in the government, but there has never been any demonstrable evidence that two governent officials can even work together, much less keep a secret. e. 99% of all conspiracy theories are delusional, but as for the remainder... f. Your character is pretty certain that they've had some sort of conpiracy related event before in their life. g. Your character firmly believes in conspiricies and *definitely* has had a such an experience in their life. h. NO thing exists that is not somehow related to Conspiricies. 21. Strength: Maximum amount of weight your character can carry before being encumbered. (Note that there may be differences between upper and lower body strength). 22. Athletic Proclivities: a. Your character's idea of exercise is getting off the couch to check the fridge. No regular exercise. b. Your character takes a walk or swims once or twice a week. Little, if any regular exercise. c. Your Character goes to the gym or otherwise works out 3-4 times a week. Moderate regular exercise. d. Your Character goes to the gym or otherwise works out daily. Intensive regular exercise. e. Your character goes to the gym or otherwise works out daily, sometimes twice daily; as well as *seriously* running in marathons or other similar competitions. Olympic level athlete to Peak human. f.
